Health informatics — Personal health data generated on a daily basis

This document provides an environmental scan of common health-related data elements that are combined with electronic health records (EHRs), patient portals and personal health record (PHR) systems. These data elements are captured in various ways, such as mobile and smart phones, and remote monitoring devices. The use of this data is applicable to a variety of healthcare-service environments.
